Fill this out and bring with you to the initial appointment to speed up the appointment. Be sure to list names and amounts of all creditors to whom you owe money. Do not mail, email, or fax to us. We need to meet with you in person to discuss your case.
If you retain us you will be asked to execute a Bankruptcy Retainer Agreement.
While I usually do not require this at the initial appointment, you may wish to fill this out before you come in to assist in advising you between chapter 7 and chapter 13.
