- Will– need to designate Beneficiaries and Guardianship for Minor or Disabled Children.
- Durable Power of Attorney (DPOA)– Who will handle your finances, and needs if you are incapacitated and can not make those decisions.
- Healthcare Surrogate- who will make your medical decisions if you are unable to do so.
- Living Will– Do you want live support? Do you want to be kept alive in a vegetative state? Are their limits? Do you want food, hydration, pain medication?
- HIPPA Release– Who has access to your medical records.
